A completed application for obtaining a foreign passport, which can be obtained from employees who accept documents.
Go through the fingerprint scanning procedure. For children, this procedure is carried out from the age of 12.
Passport of a citizen of Ukraine (upon reaching the age of 14) and Taxpayer Identification Number (TIN).
Birth certificate or similar document for children under 14 years old.
Certificate of registration of a person as a citizen of Ukraine, if the child's parents (one of the parents) were foreigners (stateless persons) at the time of his birth.
A receipt for the payment of the administrative fee or a document on exemption from its payment.
Photographs 10x15 cm. Photographs of children over 12 years of age are taken on site, and for children under 12 years of age, photographs are submitted with the application.
A previous foreign passport, including invalid ones, may also be required.
Be sure to take into account that Ukrainians over 16 years of age must submit documents in person, and for children aged 12 to 15 years, their parents or guardians submit documents. Children under the age of 12 do not necessarily have to be present. To obtain a passport for traveling abroad to an orphan child, a child deprived of parental care who has not reached the age of 16, the applicant must submit the following documents, as well as an original or certified copy of one of the following documents:

Agreement on the organization of activities of a family-type children's home.
