A child's birth certificate is an official document that confirms the fact of his birth. A birth certificate is a very important document that can be used in a variety of situations, such as school registration, receiving medical services, filing for parental leave, and providing documentation for migration or citizenship.
To obtain a duplicate birth certificate in Ukraine, you need to contact the relevant state institution that issues these documents. In order to order a duplicate birth certificate, you usually need to contact the Civil Registry Office (CRC).
Here are the steps you can take to get a duplicate birth certificate:
Contacting the relevant RAC: Find out which RAC is responsible for your place of birth. To do this, you can call the local administration or find this information on the Internet.
Filling out the application: You may need to fill out a special application for the issuance of a duplicate birth certificate. Sometimes this form can be found online on the official website of the institution concerned.
Presentation of documents: You will usually need to present some identification documents, such as a passport or an identification code.
Paying the cost: There is often a fee for a duplicate birth certificate, so make sure you are prepared to pay the cost in advance.
Waiting: After you submit your application and payment, it may take some time for your request to be processed.The process can usually take several days or even weeks, depending on the workload of the civil registry office.
If you need to obtain a duplicate birth certificate by proxy, here are the steps you can follow:
Execution of power of attorney: First, you need to execute a power of attorney in the name of the person you want to authorize to obtain a duplicate birth certificate. The power of attorney must be signed by you (the principal) and sealed with your signature, as well as the signature of a notary public.
Presentation of documents: The person representing you by power of attorney must present the original power of attorney and a copy of your identification document (for example, a passport or ID card) to the Civil Registry Office (CRC) where you were registered at birth.
Filling out the application: Together with the original power of attorney and a copy of your identification document, the representative must fill out the application for the issuance of a duplicate birth certificate.
Payment of the cost: The cost of the duplicate birth certificate can also be paid by the representative using funds that you have provided or that you have transferred to him.
Waiting and issuing: After submitting the documents and paying the cost, the RAC will process your request. When the document is ready, your representative will be able to collect the duplicate birth certificate according to established procedures.
Does the duplicate invalidate the original birth certificate. Cancellation of the original birth certificate only occurs when it is deemed invalid or when you receive a new original birth certificate.Obtaining a duplicate birth certificate does not automatically invalidate the original.
The use of a duplicate usually becomes necessary in cases of loss or damage to the original. The duplicate has the same legal force as the original and can be used in all the same situations. However, you should avoid using both documents at the same time for the same purpose, as this may create confusion or identification problems.

You can also contact the Ministry of Foreign Affairs of Ukraine with an application or questionnaire. However, it should be noted that the process of obtaining documents through the Ministry of Foreign Affairs may take longer, as it usually occurs through connections between government institutions.
