Report on the receipt of a certificate of the circumstances of the injury

Obtaining a certificate of the circumstances of the injury is necessary for its further application for a lump-sum allowance or for obtaining the status of a person with a disability as a result of the war.

The certificate is formed by the commander of the military unit from the report of the victim and the conclusion of the military medical commission, after the military personnel undergoes a military medical examination.

What is the purpose of an injury report?

A certificate of the circumstances of the injury is necessary, first of all, to fix the state of your health. It can be submitted to a medical institution where a serviceman is undergoing treatment or rehabilitation.

By analogy, a certificate is added to the report for the payment of one-time financial assistance to military personnel:

whose disability has occurred as a result of an injury (concussion, trauma or injury) received by him in the performance of military service duties or as a result of an illness associated with the performance of his military service duties, or the establishment of a disability to a person after his dismissal from military service as a result of these reasons;

whose disability occurred during the period of military service or as a result of an illness associated with his military service, or the establishment of disability for a person dismissed from military service no later than three months after her dismissal from military service or after a three-month period, but due to illness or an accident that occurred during the period of passage of the specified service.

What should you pay attention to when writing a report?

The report must contain a description of the injury received, namely information on the date, place and circumstances in which the soldier received this injury.

In addition, it is necessary to make a reference to the appendix, according to which the reference sample is defined, namely Annex 5 to the Regulations on military medical examination in the Armed Forces of Ukraine, approved by Order of the Minister of Defense of Ukraine No. 402 dated 08.14.2008.

The following should also be added to the report:

A copy of the discharge summary;

A copy of the conclusion of the military medical commission;

Explain the circumstances of the injury.
