The report for receiving a lump-sum allowance due to partial disability has become a typical document during military service.
The report is the basis for making such a payment, but it is rather a summary of all the necessary documents than a direct basis for making such a payment.
Payments of lump-sum cash benefits are carried out on the basis of an order to the commander of a military unit after receiving such a report. The order must contain information on the implementation of the necessary payment, if necessary, to obtain an extract from such an order in the future.

Amount of one-time financial assistance
From the moment of partial disability without the establishment of disability or with the appointment of disability, the soldier acquires the right to receive a lump-sum allowance.
At the same time, military personnel whose disability occurred during the period of military service or as a result of an illness associated with their military service or as a result of an illness or accident that occurred during the period of service, the allowance is paid in the amount of:
120 times the subsistence minimum established by law for able-bodied persons as of January 1 of the calendar year - for disabled people of the 1st group (UAH 297,720);
90 times the subsistence minimum established by law for able-bodied persons as of January 1 of the calendar year - for disabled people of the 2nd group (UAH 223,290);
70 times the subsistence minimum established by law for able-bodied persons as of January 1 of the calendar year - for disabled people of the 3rd group (UAH 173,670).

What documents are required to receive a lump sum cash payment?
Documents must be added to the report confirming your right to receive a lump-sum cash benefit in connection with the partial or complete disability of a serviceman:
Copy of passport;
A copy of the taxpayer identification number;
A copy of the document certifying the causes and circumstances of the injury (concussion, injury or injury), in particular that it is not related to the commission of a criminal or administrative offense by a person or is not a consequence of his actions in a state of alcoholic, narcotic or toxic or intentional infliction personal injury;
A copy of the decision of the MMC;
Original MSEC certificate;
Copy of bank account details.
