17.06.2022 13:16:24 Нормативно по п.6.2 наказу МОУ 14.08.2008 №402 https://zakon.rada.gov.ua/laws/show/z1109-08#top на військовослужбовців, які направляються на медичний огляд ВЛК у мирний час (у воєнний час почему-то нет перечня, значит следует придерживаться перечня в мирний час), подаються: Направлення на медичний огляд ВЛК; медична книжка; військовий квиток; службова та медична характеристики, але вони не є обовязковими у воєнний час; на осіб, які направляються на огляд з приводу поранень, травм, контузій, каліцтв, одержаних у період проходження ними військової служби, - оригінал довідки про обставини одержання травми (поранення, контузії, каліцтва); на учасників бойових дій - відповідні посвідчення, витяги із особових справ, інші довідкові матеріали (допускається надання ксерокопій, завірених відповідними посадовими особами та герб.печаткою в/ч).
