11.03.2024 15:47:23 На перетин кордону мають право особи, які здійснюють постійний догляд за особами з інвалідністю I чи II групи і супроводжують таких осіб для виїзду за межі України, за наявності: - документів про отримання компенсації на догляд - або документів, що підтверджують інвалідність, та акта встановлення факту здійснення догляду. Акт встановлення факту здійснення догляду за особою з інвалідністю I чи II групи складається на підставі звернення особи з інвалідністю I чи II групи або особи, яка здійснює догляд, до районної, районної у мм. Києві та Севастополі держадміністрації, виконавчого органу сільської, селищної, міської ради із заявою про здійснення особою такого догляду. Не пізніше ніж протягом п’яти робочих днів після надходження заяви складається акт встановлення факту здійснення догляду в довільній формі. Зазначений акт надсилається заявнику або видається особисто за його бажанням.

Для перетину потрібно: 1. закордонні паспорти; 2. рішення опікунської ради про встановлення опіки; 3. медичні документи, які підтверджують інвалідність
