26.03.2021 16:23:10 Доброго дня, засвідчую вам свою повагу. Отримати статус учасника бойових дій модливо 2 способами. Перший - дочекатися доки ваші документи направлять із військової частини на розгляд комісії з питань розгляду матеріалів про визнання учасниками бойових дій утвореними в Міноборони, Нацгвардії. Другий варіант - направити документи самостійно - це документи про безпосереднє залучення до проведення АТО/ООС, витяги з наказів наприклад керівника антитерористичного центру при СБУ, довідки про участь. Механізм визначений постановою кабінету міністрів України 413 від 20.08.2014. Для добровольців (у складі таких формувань, які було згодом включено до сил та засобів ато/оос) також слід додати нотаріально завірені свідчення осі- свідків з числа учасників ато/оос, які разом з вами брали участь в ато та отримали статус убд про те що ви також приймали участь в проведенні вказаних заходів

29.03.2021 16:25:12 Документи для отримання статусу УБД подаються до Адміністрації Державної прикордонної служби. Перелік документів: рапорт військовослужбовця щодо надання статусу учасника бойових дій довідка про безпосередню участь особи в здійсненні заходів із забезпечення національної безпеки і оборони, відсічі і стримування збройної агресії Російської Федерації в Донецькій та Луганській областях, забезпечення їх здійснення; завірені копії 1, 2, 3 сторінок та сторінки з місцем реєстрації паспорту, для біометричного паспорту копію паспорту та витягу з Єдиного державного демографічного реєстру щодо реєстрації місця проживання; завірена копія картки фізичної особи – платника податків; згода на збір та обробку персональних даних; витяг з наказу керівника органу охорони державного кордону (загону морської охорони) про зарахування до списків особового складу та на всі види забезпечення; витяги з наказів Генерального штабу Збройних Сил України щодо включення військовослужбовця до складу Збройних сил України.
