15.02.2023 13:23:02 Отримати статус УБД: за зверненням командира частини, який подає документи на підлеглих військовослужбовців; за особистим зверненням військовослужбовця; за особистим зверненням особи, яка звільнена з військової служби, через територіальний центр комплектування та соціальної підтримки. Необхідно надати Комісії з питань розгляду матеріалів про визнання учасниками бойових дій довідку про безпосередню участь особи у заходах, необхідних для забезпечення оборони, В обов’язковому порядку також слід подати не менше як один з перелічених документів (копії): витяги бойових донесень; витяги з журналів бойових дій (оперативних завдань, ведення оперативної обстановки); витяги з вахтових журналів; копії матеріалів спеціальних (службових) розслідувань за фактами отримання поранень, контузій, каліцтв. Додатково можуть надаватися й інші документи, які містять докази та підтверджують факт виконання бойових (службових) завдань.
