Вітаю, згідно п. 22 Постанови КМУ № 905, від 8 жовтня 2008 року, усиновлювачі мають надати визначений перелік документів, серед котрих і висновок про стан здоров’я кожного заявника (за встановленою формою), раджу ознайомитись із повним наведеним переліком документів,що є обов'язковими для всиновлення дитини, котирий наведено у п. 22 зазначеної Постанови.

Заключение о состоянии здоровья каждого заявителя по форме согласно приложению 3 к Порядку осуществления деятельности по усыновлению и осуществлению надзора за соблюдением прав усыновленных детей. Доброго дня! Особа, яка бажає усиновити дитину, подає до суду заяву про усиновлення. Також до заяви додаються:
- копія свідоцтва про шлюб, а також письмова згода на це другого з подружжя, засвідчена нотаріально, - при усиновленні дитини одним із подружжя;
- медичний висновок про стан здоров’я заявника;
- довідка з місця роботи із зазначенням заробітної плати або копія декларації про доходи;
- документ, що підтверджує право власності або користування жилим приміщенням;
- інші документи, визначені законодавством.
Судові витрати, пов’язані з розглядом справи про усиновлення, сплачуються заявником.
