13.07.2022 11:49:50 Для відновлення посвідки вам потрібно буде довести, що ви її мали і отримали її законним шляхом. Механізм відновлення схожий до процедури обміну посвідки або продовження строку її дії. Якщо ви загубили або пошкодили цей документ, вам треба буде одразу ж повідомити про це органи внутрішніх справ і ДМС України. Після цього, цей документ втрачає дійсність і знищується, якщо його знайдуть. Ви ж зможете отримати нову посвідку. Отримати нову посвідку на заміну втраченій, вам потрібно буде звернутись за нею у відділення ДМС по місцю проживання. Туди ви маєте подати певні документи. Ось їх список: Паспорт або документ, який може його замінити і його копія; Копія втраченої посвідки або оригінал пошкодженої посвідки; Документи, за якими ви отримали посвідку в перший раз; Квитанції про те, що ви сплатили вартість послуги, бланку і державне мито; Дві фотокартки розміром 3,5 на 4,5. Також вам треба буде написати заяву. Її зразок ви можете знайти в Інтернеті або попросити у відділенні ДМС.
