У заяві на реєстрацію до CEIDG необхідно вказати: ім’я, прізвище, імена батьків, дата та місце народження вид, серія та номер документа, що посвідчує особу PESEL, якщо у вас є польське громадянство або якщо він наданий на відповідних законних підставах номер NIP та REGON, якщо вони є адреса проживання та інші адреси, що стосуються передбачуваної підприємницької діяльності коди PKD (аналог КВЕД) кількість працівників, яких ви плануєте найняти дата початку діяльності інформація про страхування в ZUS, KRUS або за кордоном реквізити податкової служби, яка знаходиться за планованим місцем ведення діяльності.
