02.01.2023 15:20:11 Для призначення компенсації непрацюючі працездатні особи, які здійснюють догляд за інвалідом І групи або особою, яка досягла 80-річного віку, подають в місцеві органи з питань соціального захисту населення заяву, форма якої затверджена наказом Міністерства соціальної політики України від 22.02.2012 № 96 “Про затвердження форми Заяви про призначення усіх видів соціальної допомоги, компенсацій, субсидій та пільг”. Крім того, до заяви додаються: паспорт або документ, що посвідчує особу одержувача державної соціальної допомоги; довідка про перебування на обліку в органах ПФУ або органах праці та соціального захисту населення — для особи, за якою здійснюється догляд; паспорт або документ, що посвідчує особу, за якою здійснюється догляд; документ, який підтверджує, що особа не працює: трудова книжка та довідка органів державної податкової служби довільної форми про те, що особа не перебуває на облікуяк фізична особа — підприємець;витяг з акта огляду медико-соціальної експертної комісії.
