Доброго дня! Вам потрібно надати локументи, які підтверджують дані обставини: лікарські документи дідуся, рішення суду про визнання його недієзндатним та документи, які підтверджують,що саме Ви є його опікуном: рішення районного суду про визнання особи недієздатною та про призначення опіки (піклування); заяву опікуна; копію паспортів опікуна та підопічного (1,2,11 стр.);копію ідентифікаційного номеру опікуна; копію довідки ЛКК на підопічного або довідки МСЕК (при наявності групи інвалідності).
