20.03.2022 01:40:00 Отримайте її онлайн за допомогою наявного у вас ЕП 1. Перейдіть за посиланням https://dpvs.hsc.gov.ua/та авторизуйтесь за допомогою електронного підпису або Bank ID. Зверніть увагу! Довідка через Bank ID лише в паперовій формі. 2. Заповніть онлайн-форму для отримання довідки. 3. Під час замовлення довідки збережіть її номер. За ним ви можете самостійно перевіряти статус готовності. 4. У полі «Спосіб отримання довідки» виберіть, у якому вигляді хочете отримати довідку: електронний варіант – довідку надішлють на електронну адресу; 5. Протягом 30 днів буде сформовано довідку, яку ви можете використати за потреби.
